Leon

Case Manager and Mental Capacity Assessor

Leon is a Case Manager and Mental Capacity Assessor, with over 16 years of experience. Leon is skilled in managing complex needs and working with children and adults. He has a proven track record of grasping the specific needs of his clients and making provisions to meet the identified needs. He works in collaboration with clients and their families, solicitors, therapists and other professionals working to support his clients. He is experienced in formulating actionable strategies to meet therapy goals and promote the well-being of his clients.

Leon’s ability to listen to clients, their families, and other carers sets him apart. He works hard to promote the quality of life and independence for his clients. Leon provides the best care and support to children and adults who he is instructed to work with. His competence, empathy, and dedication make him a valued case manager and mental capacity assessor.

Leon’s training and experience as an Approved Mental Health Professional equip him with the skills to carry out complex assessments. Leon understands the complexities of decision-making and the frameworks that guide the assessment of capacity. He understands the needs of people with impaired cognition and those with communication difficulties.

Dr Shah

Consultant Psychiatrist

Qualifications:

  • Dual CCT General Adult and Older Adult Psychiatry with endorsement in Liaison Psychiatry (on the GMC Specialist Register)
  • MRCPsych Royal College of Psychiatrists
  • MB BS Guy’s King’s & St. Thomas’ School of Medicine, Kings College London.
  • BSc Upper Second Class Honours in Neuroscience & Neuropsychology with Basic Medical Sciences awarded by University of London.
  • A-Levels Awarded ‘A,A,A,B’ at Aylesbury Grammar School, Buckinghamshire
  • Approved Clinician status approved by London Regional Panel.
  • Approval under Section 12(2) of the Mental Health Act (renewed in 2017).

Brief synopsis:

Dr Rajendra Shah is an experienced Consultant Psychiatrist who has gained a great deal of success in his field of expertise. Working with a wide variety of people who have varied mental health concerns, physical disabilities, learning challenges, and brain injury is where he shines as an expert. He possesses experience in working with clients on the autism spectrum.

In accordance with the Mental Health Act, Dr Shah has been granted the status of Section 12 Approved Clinician. This gives him the authority to make judgements on the treatment of patients who are suffering from mental health conditions. In addition to this, he is a Specialist Mental Capacity Assessor and is qualified to assess an individual’s capacity for decision-making. Dr Shah approaches his profession with compassion and a focus on the patient, and he makes it his mission to deliver the highest possible standard of care for his patients at all times.

Amy

Helen

Head of Customer Referrals

Helen, is the head of our customer referrals. She has years of experience in supporting and ensuring that people using our services have a smooth experience. Helen is known for her compassion and empathy when dealing with vulnerable clients and their relatives who at times may be dealing with complex situations and distress

Helen understands that mental capacity is a sensitive issue and she goes above and beyond to ensure that every referral is handled with care and professionalism. Helen is highly knowledgeable about mental capacity and case management and works closely with solicitors and other professionals who refer clients to OFH Care. Her expertise, efficiency and understanding of the needs of these professionals make her an invaluable asset to our team, ensuring that every client receives the highest level of care and attention. With Helen at the helm of the referrals department, clients and their loved ones can rest assured that they are in good hands.

Brief synopsis:

Louise is an experienced and hardworking case manager who has more than 16 years of experience working as a paediatric nurse. Louise is a valued member of our team. She has a strong interest in providing case management services while applying her nursing skills and knowledge to achieve the best outcomes for the people she works with. The care Louise provides is based on evidence and best practice when working with children and young people in a variety of settings, including hospitals, homes, and schools.

She manages client’s needs with initiative and efficiency, integrating excellent leadership qualities with an empathic and professional approach to the specific situation of everyone she works with. Louise is of the opinion that efficient teamwork across the MDT is absolutely necessary in order to ensure the delivery of case management services and care that is both safe and of a high quality to her clients and their families.

With her wide range of work experience in nursing, including orthopaedics, learning disabilities and chronic and life-limiting conditions, she has gained skills in carrying out specialist assessments, creating, implementing, and managing care plans and providing staff training. Louise is an empathetic, compassionate, and supportive communicator with a strong ability to adapt to new environments quickly. She has a strong ability to problem solve, use her initiative, and create team morale in compromising scenarios.

Simon

Simon is a skilled Case Manager and Mental Capacity Assessor. He works with clients of all ages, including children. He has a wealth of knowledge working with different clients, including physical disabilities, neurological diseases, learning difficulties, brain injuries, mental health needs, stroke, Autistic Spectrum, Multiple Sclerosis, Alzheimer’s and dementia.

Simon is a compassionate and skilled case manager and mental capacity assessor. He has a wealth of experience managing complex needs in a client-centred way.

Simon’s training and experience as a Social Worker have equipped him with the knowledge and skills needed to effectively assess needs and decision-making abilities. Simon has worked in a variety of settings, including rehabilitation settings educational placements, schools, statutory services, hospitals, care homes and in the voluntary sector.

Simon is skilled in building strong relationships with his clients and their loved ones. He takes the time to understand their unique needs and challenges of each client, and he works closely with them to develop personalized care plans that support their goals and aspirations.

Simon is highly skilled in managing complex cases, including those with multiple and competing needs. He is able to coordinate a range of services and resources, including therapies, medical care, social support, accommodation, transportation and financial assistance, to ensure that his clients receive the best possible outcomes.

Vicky

Case Manager and Mental Capacity Assessor

Vicky is a skilled Mental Capacity Assessor and Case Manager. She has a breadth of experience working with young people and adults with complex and challenging needs including brain injury, complex physical injuries, severe and enduring mental health conditions, trauma, substance misuse issues, eating disorders, perinatal needs and complex physical health conditions. Vicky has experience working with people within the autistic spectrum, sensory loss and those receiving end-of-life care.

Vicky is highly skilled in assessing mental capacity, identifying needs and establishing care plans that promote wellbeing and recovery. She is committed to working with people who have suffered life-altering injuries or diseases and is driven by her desire to assist those she works with in achieving the most positive results that are possible for them. Her approach is person-centred, meaning that she focuses on the distinct requirements and objectives of each individual client. Vicky is a dedicated professional who is committed to giving the highest degree of care and assistance to her clients.

Lynsey

Mental Capacity Assessor

Qualifications:

  • Diploma of Higher Education in Social Work

Brief synopsis:

Lyndsay is an experienced Mental Capacity and DoLs (Deprivation of Liberty Safeguards) Assessor. She possesses knowledge and skills in assessing and managing the needs of young people and adults with varying cognitive functioning impairments, such as learning disabilities, Autism, Dementia and other mental disorders.

Lynsey is an experienced Social Worker, registered with Social Work England (SWE). She has 16 years post qualifying experience and she has over 10 years experience as an Advanced Practitioner. She is a registered Practice Educator (Stage2), ASYE assessor and Best Interest Assessor. She is the lead advisor for MCA 2005 and Community DoLS.

Lynsey is an experienced mental capacity assessor with a passion for improving the lives of vulnerable individuals. Lynsey has a deep understanding of the complexities of mental capacity and the challenges faced by individuals living with cognitive impairments.

She takes a sympathetic attitude, constantly making an effort to involve the people she works with in the decision-making process and to understand their views and wishes.

Lynsey has in-depth understanding of the legal frameworks surrounding capacity assessments, including the Mental Capacity Act 2005 and Deprivation of Liberty Safeguards. She works hard to provide comprehensive and accurate assessments in line with the relevant legislation.

Libby

She ensures that each person she works with receives individualised treatment that is tailored to their specific needs. Libby enjoys assisting her clients to accomplish the goals and objectives that are important to them.
